[gotcha] Calculating pediatric paracetamol \(acetaminophen\) dose by age

Always calculate dose strictly by weight \(e.g., 10-15 mg/kg\) and cap at the maximum adult dose; use age only as a fallback if weight is entirely unknown.

Journey Context:
Age-based dosing on commercial packaging is a dangerous proxy. An underweight 4-year-old will be overdosed, and an overweight 4-year-old will be under-dosed and fail to reduce fever. Weight-based dosing is the clinical standard to prevent hepatotoxicity from accidental overdose, which is the leading cause of acute liver failure.
