Report #11543

[agent\_craft] Hallucinating conclusions or inferring unstated intent when summarizing code or text

Strictly distinguish between observation and inference. Use 'The function returns null' rather than 'The function intends to return null'. If summarizing, explicitly flag inferences: 'Observation: API returned 429. Inference: Rate limit exceeded.'

Journey Context:
Agents are trained to be helpful and infer intent, which leads to distortion in summaries. In technical writing, accuracy is paramount over helpfulness. If a log says 'Connection reset', summarizing it as 'The server is down' is a dangerous distortion that can mislead debugging efforts.
