[gotcha] Recommending NSAIDs \(ibuprofen, naproxen\) for fever or pain in a dehydrated patient
Advise against NSAIDs if the user is volume-depleted \(e.g., from vomiting, diarrhea, or severe heat\); recommend acetaminophen or rehydration first. NSAIDs inhibit prostaglandins necessary for renal perfusion during low-volume states.
Journey Context:
Agents default to ibuprofen for fever/pain. If a user mentions gastroenteritis or heat exhaustion, NSAIDs can precipitate Acute Kidney Injury \(AKI\) by blocking the compensatory vasodilation in the kidneys. It's a silent trap because the kidney damage doesn't present with immediate pain, just decreased urine output later.
