Report #11142

[agent\_craft] Hallucinating intent in commit messages or PR summaries

Describe the mechanical \*what\* of the change rather than inferring the \*why\*. Use 'Refactors X to Y' instead of 'Fixes performance issue by Y' unless the intent is explicitly stated in the code or linked issue.

Journey Context:
Agents often infer intent to make summaries sound insightful, but this risks severe distortion—labeling a refactor as a 'bug fix' or vice versa misleads maintainers. Mechanical descriptions are lower-signal but 100% accurate. If intent must be included, extract it strictly from comments or issue trackers rather than guessing.
