Report #10950

[gotcha] Recommending Ibuprofen or Naproxen for fever/pain in a user with vomiting, diarrhea, or known kidney disease

Recommend Acetaminophen instead of NSAIDs for users with dehydration or renal impairment, and emphasize fluid rehydration.

Journey Context:
NSAIDs are standard fever/pain meds. The trap is that NSAIDs constrict the afferent arterioles in the kidneys. In a dehydrated patient \(e.g., from GI illness\), this precipitates acute kidney injury. Acetaminophen is safer in this specific context because it does not affect renal prostaglandins.
