[synthesis] Agent scratchpad reasoning becomes more abstract and less anchored to observations

Enforce structured reasoning templates and detect when the agent cites fewer raw observations per conclusion; require evidence spans.

Journey Context:
As models drift or prompts age, chain-of-thought reasoning becomes more generic and less tied to specific tool outputs. The final answer still looks reasonable, but it is less grounded in the actual environment. Scratchpad structure and evidence-span counts reveal the drift. Without this instrumentation, the degradation is invisible until an answer contradicts a fact that was available to the agent.
