[synthesis] Retrieved examples degrade to semantically similar but wrong precedents

Evaluate retrieved-case outcome match, not just embedding similarity; log when the agent reuses a precedent whose recorded outcome was failure or override.

Journey Context:
Vector retrieval returns historical cases that look like the current situation but had different root causes or were later overridden. Embedding similarity cannot detect outcome mismatch. The agent's reasoning and output look coherent, so the recommendation is wrong in a way that is hard to spot. Tracking precedent outcome labels and human override flags catches the degradation before it propagates into the agent's decision.
