[synthesis] Self-correction loop reinforces the wrong hypothesis

Require independent evidence for every correction, cap self-correction iterations, and escalate to a different model or human after the threshold.

Journey Context:
Reflexion showed that self-correction can help, but when feedback is ambiguous the model can entrench a wrong hypothesis by using its own explanation as evidence. This is the confirmation-bias trap familiar from human reasoning. Independent evidence such as test results, lint output, or git diffs, plus a hard iteration cap, breaks the loop.
