[gotcha] Model keeps calling the wrong MCP tool or refuses to call the right one

Use unique, action-oriented names \(\`search\_issues\`, not \`query\` or \`get\_data\`\). Descriptions must state when to use the tool and when not to. Avoid overlapping capabilities; merge tools that differ only slightly. Test with a small set of golden prompts and iterate descriptions before prompt engineering.
