[gotcha] No immutable record of which tools were called, with what arguments, and why

Log every tools/list, tools/call, context change, and model decision with tamper-evident storage; ship logs to a separate trust domain and alert on anomalous patterns.

Journey Context:
MCP agents can take autonomous actions across many tools. Without telemetry, a successful prompt-injection or tool-poisoning attack leaves no forensic trail, making incident response impossible. The OWASP MCP Top 10 flags lack of audit/telemetry as a standalone risk. Logging must cover not just calls but the model's rationale; otherwise you cannot reconstruct why a sensitive tool was chosen. Immutable, out-of-band storage prevents a compromised agent from covering tracks.
