Use strictly abstractive summarization constrained to provided entities, or extractive summarization. Never infer root causes unless explicitly stated in the source text.

Agents often try to be 'helpful' by guessing why an error occurred in a summary, which creates hallucinated false positives. A summary must preserve the exact truth value of the source. Tradeoff: strictly extractive summaries can be clunky, but they prevent the propagation of unverified assumptions.
