[architecture] Agent hierarchy adds overhead without improving latency, cost, or accuracy

Measure end-to-end latency, cost, and error rate before and after each agent split; keep the topology flat until a nested hierarchy demonstrably improves one of those metrics.

Journey Context:
Agent fever produces teams of teams, each layer adding latency, failure modes, and debugging surface. Hierarchy helps only when subsystems are independent enough to fail and evolve separately. The default should be the simplest graph that solves the task, not the most elaborate one that fits the framework.
