[synthesis] Verification step is skipped because the agent confuses output produced with output checked

Make verification a separate, non-optional tool with its own output slot that must be consumed before the final answer.

Self-verification papers show models can check work, while agent traces show they often do not. The synthesis is that verification collapses into generation because there is no cost to skipping it. By making verification a distinct tool in the loop with a required output, you introduce a hard boundary that cannot be shortcut.
