[synthesis] Agent abandons the plan at the first obstacle because planning and execution share the same context budget

Keep a separate, protected plan buffer that cannot be evicted by execution chatter, and refresh it only at explicit replanning events.

Long-context research shows middle-position degradation, while agent papers show replanning under uncertainty. The synthesis is that when the plan lives in the same context as tool outputs, detailed observations push the plan out. The agent then improvises. A protected plan buffer preserves intent, and refreshing it only deliberately prevents plan erosion.
