[synthesis] Partial success masks total failure when subtask completion is reported as goal completion

Define a mandatory goal-integrity check that compares the final artifact against the original request, not against the last subtask's success metric.

The synthesis is that subtask success creates a local reward signal that drowns out global failure: the agent reports 'file written' when the goal was 'fix the bug', or 'tests ran' when the goal was 'tests pass'. A final integrity check against the original request prevents this.
