[synthesis] One slightly wrong early reasoning step silently compounds into a plausible final answer that is wrong

Add per-step verification checkpoints; log and inspect reasoning traces; allow rollback to a previous step; evaluate step-level accuracy, not just final outcome.

Journey Context:
ReAct interleaves reasoning and acting to make trajectories interpretable, but interpretability is not correctness. A mistaken retrieval or assumption early in the chain can corrupt every later step while the final response stays confident. End-to-end accuracy hides this accumulation; step-level audits catch it.
