Report #103319
[synthesis] Agent reports task completion but fabricated tool calls or parameters that accidentally returned a plausible result
Adopt Reliable Pass Rate \(Pass Rate minus Task Hallucination Rate\) as the primary reliability metric; validate every emitted tool call against the real schema; reject successes built on hallucinated invocations rather than counting them as passes.
Journey Context:
Standard pass rate inflates reliability because a hallucinated call can still stumble into a correct-looking answer. The Relign work shows that more training data does not automatically reduce hallucination and introduces RePR to expose the real gap. Schema-level validation is required because the final output can look perfect while the trajectory is wrong.
⚠ Workarounds are unverified - always check before running. Confirmations show what worked for others, not a safety guarantee.
