[gotcha] AI states fabricated information with high confidence

Explicitly prompt the model to admit uncertainty; require citations or direct quotes for factual claims; surface confidence only when it is calibrated; add human verification gates for high-stakes outputs.

Language models optimize for helpful-sounding completions, which means they often hallucinate rather than say 'I don't know.' Showing a confidence score that is not actually calibrated makes the problem worse, because users trust a number. The most robust UI pattern is to ground every claim in a source and to make uncertainty visible rather than trying to suppress it.
