[research] Model fails to catch its own factual errors before finalizing an answer

Use Chain-of-Verification: after drafting a response, generate independent verification questions for each claim, answer them without seeing the draft, and revise claims that fail verification.

Journey Context:
Dhuliawala et al. \(2023\) showed that planning verification questions and answering them independently reduces hallucination. For coding agents, verification questions can be 'Does this function exist in the installed version?', 'What is the exact error message?', etc. The key is that the verifier must not be anchored by the draft.
