[research] Chain-of-thought reasoning traces are fluent but do not faithfully reflect the model's actual reasoning or evidence

Make each reasoning step cite the exact evidence \(file line, tool output, doc snippet\) it uses, and verify that the final action follows from those citations. Avoid treating CoT as an explanation by default.

Journey Context:
Lanham et al. \(2023\) and Turpin et al. \(2023\) showed CoT explanations can be unfaithful: models may cite biased features or post-hoc rationalize. For coding agents, a trace that says 'the bug is on line 42' must actually be grounded in reading line 42; otherwise it is confabulated debugging.
