Break the output into atomic facts and validate each one against an authoritative source. Report which atoms are supported, unsupported, or contradicted rather than giving a single binary verdict.

A whole paragraph can look right while one clause is invented; atomic verification catches the invented clause. This is especially important for code explanations and design rationales.
