[research] Agent handoffs in multi-agent systems fail or route to the wrong sub-agent

Add span-level handoff metrics: track planning accuracy, handoff accuracy, and collaboration success rate; log the orchestrator's routing decision and alternatives on the trace, and evaluate handoffs with a dedicated capability suite before adding more agents.

Journey Context:
Multi-agent failures are often coordination failures, not model failures. Generic end-to-end metrics cannot tell you whether the orchestrator chose the wrong specialist or whether two agents duplicated work. Handoff-specific span attributes and evals are needed, plus human review for emergent coordination issues.
