[counterintuitive] Hallucinations will disappear with more RLHF, data, or larger models

Treat hallucination as a residual risk, not a fully fixable bug. Ground responses in retrievable evidence, use atomic fact verification, calibrated abstention, and never present unverified model outputs as authoritative.
