[gotcha] Interaction checker covers only prescription drugs and misses food or supplement interactions

Always query grapefruit, St. John's wort, vitamin K, ginkgo, and high-dose fish oil when the user takes warfarin, statins, anti-retrovirals, or immunosuppressants; route any suspected interaction to a pharmacist.

Journey Context:
Prescription-only interaction APIs give a false sense of completeness. Some of the highest-risk interactions are with foods and OTC supplements that patients rarely report unless prompted. The fix is to ask explicitly about a short, high-yield list rather than relying on a database join of Rx records.
