[gotcha] Agent scales adult drug doses linearly by a child's body weight

Reject simple proportional scaling; require pediatric weight-band or body-surface-area protocols from a pediatric formulary, and always escalate to a pediatric prescriber or pharmacist before finalizing any dose.
