[synthesis] Agent returns plausible-looking output while a critical subtask failed silently

Define success as a list of verifiable assertions \(tests, schema checks, diff assertions, existence checks\) and fail closed if any assertion fails. Do not accept natural-language summaries as completion evidence.

Demos optimize for a coherent final answer; production requires invariants. Anthropic's SWE-bench work stresses that code agents need automated verification. The dangerous case is when 80% of steps succeed and the final summary omits the 20% that failed. Assertion-based completion criteria force the agent to surface gaps instead of smoothing them over.
