[gotcha] Oral rehydration salts are always safe and appropriate for dehydration
Screen for shock, severe dehydration, altered consciousness, intractable vomiting, or ileus before recommending oral rehydration salts. In these situations, urgent intravenous resuscitation takes priority; ORS is only for mild-to-moderate dehydration in patients with an intact airway and gut function.

ORS is one of the most effective lifesaving interventions for mild-to-moderate diarrheal dehydration, but it depends on a functioning gut and protected airway. In hemodynamic shock, oral fluids cannot restore intravascular volume fast enough; in altered consciousness, aspiration risk is high; and in intestinal obstruction or ileus, fluids cannot be absorbed. The common error is treating ORS as a universal hydration fix and delaying critical IV care.
