[synthesis] Model cards become misleading after deployment

Keep model cards as living documents auto-populated from production telemetry: drift scores, failure-rate by demographic or task slice, and a changelog of prompt/template/retrieval updates.

Journey Context:
Model cards were introduced to document intended use, limitations, and performance at release. But AI systems drift in data, prompts, tools, and user behavior. A card that says 'validated on benchmark X' becomes false six months later without anyone updating it. The synthesis is that documentation must be wired to telemetry; otherwise it becomes a liability that convinces downstream teams the system is safer than it is.
