[synthesis] Context-window pressure drops the original user constraint while the agent keeps iterating confidently

Pin non-negotiable requirements in an external artifact and re-read it before every edit cycle; never rely on memory for invariants.

Journey Context:
As the conversation grows, critical constraints migrate out of the active context window. The agent then optimizes for local coherence \(the last few turns\) and quietly violates the original goal. A common wrong fix is to summarize constraints inline, which itself gets dropped. An external file or dedicated tool state forces a re-read and acts as a guardrail that survives context eviction.
