Report #101303

[synthesis] Why do some agent modes call tools accurately while others hallucinate invocations or loop forever?

Treat tool descriptions and system prompts as a single co-evolved design surface: refine them together, run automated evals, and plan per-model prompt tailoring when supporting multiple LLMs.

Journey Context:
The VS Code team building Copilot Agent Mode wrote that a large share of development time went into refining tool descriptions and the system prompt together, using automated evaluations, and that prompts will be tailored per model as more LLMs are supported. The synthesis is that reliable tool use is a prompt-engineering and evaluation problem, not just a model capability: the schema definitions and the instructions must be designed as one system so the model knows when and how to invoke each tool.
