[frontier] Long sessions accumulate noise until the agent's outputs degrade

Adopt a Plan → Execute → Reset discipline: force a planning phase before generation, execute incrementally, and treat periodic session reset as a high-leverage control, not inefficiency. Pair with meta-prompting that requires stating assumptions before acting.

Journey Context:
Context degradation is structural, not a prompt-engineering bug to out-prompt. Planning prevents premature assumptions from propagating; stepwise execution keeps the working context focused; resetting removes accumulated noise and re-establishes correct prioritization. Meta-prompting introduces friction before the model commits to an interpretation, which is where hallucinations and drift usually start.
