Report #100889

[gotcha] Users stop verifying polished AI suggestions, then miss errors in high-stakes outputs

Keep humans in the loop for consequential actions. Use structured outputs and explicit confirmation steps; show disconfirming evidence \(what the model is unsure about\); log overrides and corrections; and avoid auto-executing destructive or irreversible operations.

Journey Context:
Microsoft's Guidelines for Human-AI Interaction include making it easy to edit, refine, or recover. Apple HIG says to ask permission before destructive tasks and give people the ability to revert. The failure mode is automation complacency: as outputs look polished, users stop checking. The antidote is graduated control and deliberate friction for high-impact decisions, not a blanket 'auto-approve'.
