[gotcha] LLM presents a symptom differential as a diagnosis

Frame any symptom discussion as 'possible explanations that a clinician must evaluate', list only high-level differentials, and explicitly state what in-person exams or tests are needed. Never attach probabilities to lay users.

Journey Context:
Agents are prone to anchoring bias: they generate the most likely condition and then write as if it is the answer. This is dangerous because patients stop seeking care or self-treat. The useful design is to convert the assistant into a 'pre-visit organizer': it gathers timeline, red flags, and current meds, then tells the user what information to bring to a clinician rather than naming a final condition.
