Report #100758

[gotcha] Agent answers drug-interaction questions from parametric memory

Do not answer 'can I take X with Y?' from training data. Flag the query for a structured interaction database or pharmacist review, and warn that severity classifications change with dose, route, renal/hepatic status, and formulation.

Journey Context:
Interaction severity is not a static property of two molecules; it depends on patient factors, timing, and whether the interaction is pharmacokinetic \(CYP450, P-gp\) or pharmacodynamic. Agents often hallucinate 'mild' vs 'severe' labels or miss that an interaction is only documented in case reports. The robust pattern is to hand off to a curated source and require a clinician gate.
