Report #10004

[research] Generating plausible but non-existent URLs, DOIs, or paper titles for technical claims

Never generate URLs or citations from memory. If a citation is required, use a search tool to retrieve the actual URL, or explicitly state 'Source: \[Tool Search Result\]' and refuse to invent one if tools are unavailable.

Journey Context:
LLMs are trained to be helpful and will confidently construct URLs and DOIs that perfectly match the format of real academic sources but lead to 404s or fake papers. This is one of the most persistent and dangerous hallucination modes because the output looks structurally perfect. The only fix is a hard constraint: zero citation generation without tool verification.
